REQUEST SUPPORT

The Lewitzke Foundation Inc. is grateful to be able to support your non-profit organization. To request funding support please complete the accompanying form in its entirety and upload the following documents:

A proposal breaking out the following details:
- Your organization's history and mission.
- A description of the program you are trying to fund, including how many people/students you serve regularly.
- The amount of funding requested, and how you will use our funds for your program
Your total program/project budget
A copy of your proof of 501(c)(3) status

WHO DO WE SUPPORT?

An applicant must be a tax-exempt organization under section 501(c)(3) of the Internal Revenue Code, and should not be a private foundation as defined in section 509(a) of the IRS code. The Lewitzke Foundation Inc. does not make grants to individuals or private businesses.

WHEN ARE FUNDS DONATED?

The Lewitzke Foundation Inc. Board of Directors meets quarterly and chooses to make donations at various times throughout the year. The Foundation will notify your organization to confirm receipt of your proposal, and then again after the Board of Directors makes its funding decisions.

HOW OFTEN CAN REQUESTS FOR SUPPORT BE MADE?

All funding decisions are based on need and The Lewitzke Foundation Inc. does not cap an organization's funding limit but does encourage applicants to develop and rely upon diverse funding sources.

IS FUNDING GUARANTEED?

Unfortunately, no. All qualified requests will be reviewed by the Board of Directors and funding decisions are based on numerous variables. If your request is denied please don't be discouraged to reapply at a later date unless notified that the organization and/or project does not qualify as a 501(c)(3) or meet the vision and standards of the Lewitzke Foundation Inc. and its Board Members.
